By completing this form, I am signing up to serve in a volunteer capacity for the 2026 Rainforest Falls! VBS program.

I understand that I will be required to have a current background check on file if I will be working with children. Forms must be submitted every two years. Someone will be reaching out to you after completing this form to get you a background check form. Filled out forms can be placed in sealed envelope and returned to Tanya Henthorn or Casey Harvey.

You will also be asked to do a Child Abuse Protection Training.
