By completing this form, I am signing up to serve in a volunteer capacity for the 2024 Scuba! VBS program.

I understand that I will be required to have a current background check on file if I will be working with children. You can CLICK HERE to obtain a background check form. Forms must be submitted every two years. Filled out forms can be placed in sealed envelope and returned to Tanya Henthorn or Casey Harvey.

You will also be asked to do a Child Abuse Protection Training.
